Florida Pathology Laboratory offers several billing options in order to meet the various needs of our clients. We will be happy to discuss the best billing option for your business. For questions regarding a patient or existing client bill, please call our Billing Department at (407) 200-2249.

Client Billing and Hospital Accounts
An individual client, client location, or hospital client may establish an account to be billed on a monthly basis. An itemized list of patients and performed tests will be sent to the physician/client office monthly. Terms of payment are net 30 days.

Third Party
The following information must be provided in order for Florida Pathology Laboratory to bill third party payors: 

▪	Complete Patient Information including Name, Date of Birth, Address, Sex
▪	Ordering Physician’s Name
▪	Diagnosis Code(s)
▪	Insurance Company’s Name
▪	Policy Number (Attach copy of Insurance Card)

Medicare/Medicaid Billing
The following information must be provided in order for Florida Pathology Laboratory to bill Medicare/Medicaid:

▪	Complete Patient Information including Name, Date of Birth, Address, Sex
▪	Ordering Physician’s Name
▪	Appropriate Diagnosis Code(s) for each test ordered
▪	Medicare/Medicaid number
▪	For Medicare, a completed Advance Beneficiary Notice form for routine screening testing or testing not considered medically necessary as indicated by Medicare criteria.
